How To Submit Your Request

Before filling out the forms, please make sure you have all the available information regarding your case, and have the scanned copies (pdf format) of all the documents to be served with you, as you will be required to upload during the online fill-out process.

Case Number.

Complete names of all the parties to the action.

Confirmed address(es) of party(ies) to be served.

Complete list of all documents to be served.

Name and address of the forum court.

Name, address, and contact number of the requesting party(ies)' counsel.

Deadline to answer the action as indicated in the documents to be served.

After filling out the online forms, you will be prompted to make a payment for our service fee. Any fees to be imposed by the central authority of the designated country, and additional shipping fees for transmittal of documents are excluded and shall be collected at a later time.

The Hague Service Request Model Form shall be generated in English language. For countries that requires translation of the Model Form, the services of a certified translator is advisable.
